We have simplified the registration
process this year by having the process on-line. Follow these 3
steps:
1.
To register for the upcoming season
you must first have a valid
USA hockey
2010-2011 registration number.
(You will know your number is a 2010-2011 number if the 4th
digit is a “1”.) The
contact information that we used to collect on our forms will be
collected via this process.

2.
Once you have obtained the
registration number, register for the LVMC Wolfpack.

The non-refundable registration fee
is $350
·

3.
After registering, print out
the ‘USA Hockey Consent to Treat form’ and send to the LVMC, PO Box
214, Long Valley, NJ 07853. Or you can scan and email the form to:

Consent To Treat Form
- The total annual cost (excluding jerseys
and tournaments) for this year are:
- Bantam: $1,200
- Peewee: $1,200
- Squirt:
$1,200
- Mite:
$1,000
- Mini-Mite: $250 registration fee. Cost will be $450 for
the season.
- Travel Players:
No registration fee is required at time of
registration. Cost
will be $100 per 5 ice sessions (practice/game)
