2009-10 REGISTRATION

Registrations must be mailed to: LVIHC, PO Box 214, Long Valley NJ 07853.

Envelope must include the Following Forms
1. Registration Form for NEW Players to the Club
1. Registration Form for RETURNING Players to the Club

2. a non refundable $250 check made payable to "LVIHC",

3. a copy of the usa hockey registration for the 2009/2010 season USA Hockey LINK

4. a completed Copy of the Consent to Treat Form Form
5. a completed copy of the Waiver of Liability Form Form

The anticipated costs for the 2009/2010 season are $400 for the mini mites, $700-$900 for the mites, and $900-$1,100 for the squirts, peewees, and bantams. The final amount will be determined once the seasons is underway and will be based on the total expenses of the program -vs- the number of players enrolled. LVCM-IHC is a not for profit entity, where all revenue is used to pay for the cost of providing a great hockey experience to our players. This year the fees will be invoiced in two payments. The first will be $250 due at registration (mini mites pay $250), and the second and final invoiced and due in January 2010.
